MIH
Mobile Integrated Healthcare or MIH is a patient-centered community-based care model that uses EMS providers, in particular paramedics in Connecticut, to deliver health services outside the hospital, and often in the home. It is intended to reduce unnecessary emergency department visits, manage chronic illnesses, and improve care transitions, which ultimately lowers costs, improve patient access to care, enhance healthy outcomes, and increase collaboration between healthcare providers.
Another term that is sometimes used when referring to MIH is Community Paramedicine. Some states make a distinction between Community Paramedicine and MIH. In Connecticut the term MIH is used to describe both types of programs.
MIH programs can vary in form and function, ranging from simple programs such as health screenings to more complex programs like hospital or primary care in the home.
EMS services may work with their local communities in partnership with hospitals, physicians, or local health districts to address their individual needs.
