The ACT (Assertive Community Treatment) Team provides clinical and case management services to individuals who have serious mental illness, often complicated by substance abuse. These individuals generally have histories of multiple psychiatric hospitalizations and/or emergency room visits, but have been unable to connect to traditional outpatient services. Team staff addresses the needs of these individuals through frequent, supportive contacts in the community. Services provided include medication delivery and monitoring, transportation to medical, probation, other appointments, assistance with the activities of daily living, efforts to facilitate involvement in meaningful social and/or work activities, and collaboration with families, landlords, and other members of the individuals social/service network. The staffs are scheduled to work on a rotating basis in order to cover all hours of operation, and a telephone on-call system provides after hours coverage. Approximately 60 unduplicated clients are served per year.

Monday through Friday from 8:30 AM to 5:00 PM
