For more details, please see CHC Contact Information What do I need to bring to my appointment? Patients must bring a valid form of ID, proof of address, and insurance information. You can apply for the Sliding Fee Scale Program by completing an application at the CHC and providing proof of income. How do I pay for my appointment at a Community Health Center? All patients are accepted regardless of their ability to pay or insurance status. CHCs accept Medicaid, Medicare, SAGA, HUSKY, and many private insurance plans. A Sliding Fee Scale Program is available to patients. Payment plans can also be arranged. What is the Sliding Fee Scale Program? This Program offers reduced prices to qualified persons and families. It is based on the Federal Poverty Level guidelines. The Program may reduce your bill 20-80%. Patients must provide proof of income (last 2 paycheck stubs, copy of entitlement checks, W2, etc.) to apply. What happens if there is an emergency after business hours? For emergencies, go to the nearest hospital emergency room or call 911. For other urgent medical problems, each CHC has a 24-hour telephone line. This program is aimed at providing quality healthcare to homeless individuals. A few examples of the services include emergency shelters, soup kitchens, food pantries, blood pressure screenings, and flu vaccine clinics.
