Certification Requirements
Before applying for licensure, please familiarize yourself with the general licensing policies.
An applicant for a community health worker may qualify for certification provided the applicant:
Is trained and educated as a community health worker by an organization approved by the Community Health Worker Advisory Body;
Is at least sixteen years of age;
Submits a professional reference from an employer with direct knowledge of the applicant's experience as a community health worker;
Submits a reference from a member of the community with direct knowledge of the applicant's experience as a community health worker, and
Has a minimum of one thousand (1,000) hours of experience working as a community health worker during the three years prior to the date of such application;
OR
Has a minimum of two thousand (2,000) hours of paid or unpaid experience as a community health worker;
Submits a professional reference from an employer with direct knowledge of the applicant's experience as a community health worker
Submits a reference from a member of the community with direct knowledge of the applicant's experience as a community health worker.
Application instructions
As part of the application, applicants will be prompted to upload the following documents:
- A recent photo of the applicant;
- Verification of experience form documenting the appropriate number of hours of paid or unpaid experience;
- A letter of reference from an employer with direct knowledge of the applicant's experience;
- A professional reference letter from a member of the community with direct knowledge of the applicant's experience as a community health worker.

Applications are only accepted online. Please select this link to apply online. The application fee is $100. After you have created your online account, select 'Initial Application' under 'More Online Services' and then expand the 'Public Health Practitioners' grouping.
