License Based on an Out-of-State License

The Department may grant a license to any applicant who is licensed as an eyelash technician in another U.S. state, the District of Columbia or a commonwealth or territory subject to the laws of the United States.

Application Instructions

A completed application.  Applications are only accepted online.  Please select this link to apply online.  After you have registered for an account, please select 'Initial Application' at the bottom left of the screen and then expand the 'Appearance Enhancement' grouping.  The application fee is $100.

As part of the online application, the applicant will upload a copy of current out-of-sate license in good standing to practice as an eyelash technician and a form documenting that the applicant has two years of licensed experience as an eyelash technician in the other state.   If the state where the experience occurred did not have a licensing program at the time the experience occurred, please upload a statement indicating same.
