The Connecticut Department of Public Health will send written verification of licenses provided the licensee completes and submits a Consent for Release of Confidential Records form prior to the Department generating and mailing a license verification.  The form may be emailed, faxed or mailed to the address indicated on the form.  There is no fee for this service.

 

Please note that the Department does not complete license verification forms from third parties.  The verification report generated by the Department contains all relevant data and is universally accepted by state licensing boards, health care facilities and HMOs.  License verifications are generally mailed within 10 business days from the date of receipt.
