Hartford – The Connecticut Department of Public Health (DPH) today announced that medical records of former patients of Carl J. Schiano, D.O., a physician formerly practicing in Naugatuck, are now available through DPH.

Dozens of Dr. Schiano’s former patients contacted DPH reporting they were unable to obtain copies of their patient records. With assistance from the Office of the Attorney General, DPH took possession of the records and cataloged them for distribution to patients.

Records will be distributed at the Waterbury Health Department, located at 1 Jefferson Square, Waterbury, CT on June 27th and 29th from 9 a.m. to 4 p.m. and July 11th and 13th from 9 a.m. to 4 p.m. Former patients of Dr. Schiano who wish to obtain their medical records should contact DPH at (860) 209-5229 and leave their name and date of birth prior to picking up their records. A photo ID will be required to release the records. Former patients with other questions may contact DPH at (860) 509-7552.

Dr. Schiano’s medical license was suspended by the Connecticut Medical Examining Board in February based on allegations related to abandonment of his practice, failure to respond to requests for records from patients, and other issues. DPH accepted the surrender of Dr. Schiano’s medical license last week.
