Apply for the Medal

1. Download the application: 

  • Connecticut Veterans Wartime Service Medal/Service member and Veteran Registry Application or
  • Posthumous Connecticut Veterans Wartime Service Medal Application

    2. Complete, sign application and include the following documents:

  • Proof of service during a period of war (i.e. DD Form 214 or other documentation if DD Form 214 is unavailable)
  • Proof of current residency in the State of Connecticut or proof of residency during time of qualifying service (e.g., photocopy of State of Connecticut driver’s license).

    3. Send the completed application and supporting documents to:
