Within six-months of the date of issuance of a school construction project Change Order or Change Directive, each district - through their design professional and/or construction manager - should submit:
- A completed and signed FORM SCG-042.CO indicating the change order value approved by the project’s building committee
- A completed FORM SCG-043.CO also known as the State Change Order Summary spreadsheet
- All hard-copy documentation required for determination of eligibility for the change order or change directive being submitted for review,
