Step by Step Guide for Guardians to Register a Minor Patient

STEP 1: Schedule Appointments with Two Physicians
  • Identify the two physicians that will confirm that the palliative use of marijuana is in the minor patient’s best interest.
    • One of the physicians must be the patient’s primary care provider.
    • The other physician must be board certified in an area of medicine involved in the treatment of the debilitating condition for which the qualifying patient will be certified.
  • One of these two physicians must log into the medical marijuana registration system and certify that the patient has one of the qualifying medical conditions listed above. The process for physician certification can be found at: http://www.ct.gov/dcp/mmp-physician
    • This physician will ask you – the caregiver – for a valid email address and primary telephone number that the Department can use to communicate with you about the minor’s registration.

  • The second physician must provide you with a letter confirming that the palliative use of marijuana is in the patient’s best interest.
STEP 2: Complete the Patient Registration Process
After your scheduled physician appointments, the minor patient should have a record in DCP’s medical marijuana registration system.  View/print the "Quick Reference Card".
The parent or guardian must complete the registration by:
  • Using the following link to set up a free account in the State’s Business Network system: https://www.biznet.ct.gov/DCP-MMRP.  This is a one-time process and will allow you access to the online medical marijuana registration system.

  • Log in to https://www.biznet.ct.gov/DCP-MMRP using the email and password you used during the Business Network registration process.

  • Select “I am a Patient” at which point you will be asked to provide the minor patient’s date of birth and the email address you provided to the certifying physician.

  • Once you are logged in, you will see a pre-filled form with the patient information entered by the physician.  If this information is not correct, you must arrange for the physician’s office to correct it.

  • After confirming the minor patient information, you will be asked to select the dispensary facility from which you intend to purchase medical marijuana.  Current dispensary facilities can be found at:  http://www.ct.gov/dcp/dispensarylist .

  • Next, you will need to provide information about yourself, as the patient’s caregiver.

  • Finally, you must select a payment method. The fee for a minor patient is $100 (starting July 1, 2023, the state registration fee will no longer be required).
STEP 3: Complete the Caregiver Registration Process
  • After completing the patient registration, you will move into the Caregiver portion of the registration system.

  • At this point, you will see a pre-filled screen with the patient information entered by the physician and the caregiver information entered by yourself.

  • You will be asked to answer a few questions confirming your understanding of the role of a caregiver and your willingness to act as a responsible caregiver.

  • Next, you will be asked to upload proof of identity, and a proof of residency.

  • You will also be asked to upload the letter from the second physician that confirms that the minor patient will benefit from the palliative use of marijuana.

  • Finally, you must select a payment method.  The fee for caregiver is $25 (starting July 1, 2023, the state registration fee will no longer be required).
STEP 4: DCP Reviews the Application
  • Once the patient and caregiver portions of the application process are complete, DCP will review the information.

  • You will receive an email confirming that your registration is approved or otherwise identifying the information that is missing.

  • DCP will mail a medical marijuana registration card for the caregiver.

  • DCP will not be making ID cards for minor patients.  The minor's e-mail confirmation and your caregiver card can be used as proof that the minor patient is registered.  In addition, law enforcement has secure access to the registration system to confirm the patient’s status.
