Forms and Reporting Documents
- Cash Advance Form - must be submitted with copies of signed vendor contracts
- Reimbursement Request Form - must be submitted with copies of associated invoices and proofs of payments
- Quarterly Progress Report - must be submitted within 30 days of the close of the quarter (March 31; June 30; September 30; December 31)
- Quarterly Financial Report - must be submitted within 30 days of the close of the quarter (March 31; June 30; September 30; December 31)
All forms and documents should be submitted to demhs.nsgp@ct.gov. Please include your organization's official name, the grant program, and award year and number on all correspondence.
