The information below outlines the steps and forms required for registering an apprentice in Connecticut, along with the associated fees and mailing instructions. All three forms must be completed and sent to the mailing address specified below.

Form 1 - Apprentice Registration Agreement

Apprentice registration agreement and terms. Please complete form and send to the mailing address specified above.

Form 2 - Journey Person List

Journeyperson names, license type and number. Please complete form and send to the mailing address specified above.

Form 3 - Registration Payment Invoice

Sponsor information required along with payment. Please complete form and send to the mailing address specified above.

Important Information Regarding your Apprenticeship Registration

To register for the apprenticeship program, please follow the steps below to ensure correct information is submitted and to prevent returned applications.

  • Please fill out forms 1 through 3 completely and render appropriate fees (links to the required forms can be found at the bottom of this page).
  • Total cost is $110 ($50 due from the apprentice and $60 due from the sponsor). Please note: If the packet is incorrect, it will be returned with the enclosed fees.
When mailing your registration information please be sure that you have enclosed:

1) Forms 1 through 3

2) Check(s) or money order(s) with the correct amount
