Important Information Regarding your Apprenticeship Registration
To register for the apprenticeship program, please follow the steps below to ensure correct information is submitted and to prevent returned applications.
- Please fill out forms 1 through 3 completely and render appropriate fees (links to the required forms can be found at the bottom of this page).
- Total cost is $110 ($50 due from the apprentice and $60 due from the sponsor). Please note: If the packet is incorrect, it will be returned with the enclosed fees.
When mailing your registration information please be sure that you have enclosed:
1) Forms 1 through 3
2) Check(s) or money order(s) with the correct amount
