Documents/Forms

The following are required documents:

1. Lease-Out Agreement or License Agreement.   
2. Contractor Authorization. A sample of a certificate or resolution acceptable to the Department of Administrative Services. Please complete and sign the document as required. Note:  A certificate that is self-serving will not be accepted.  
3. Contact Affidavit.  Your signature and acknowledgement before Commissioner of the Superior Court or Notary Public is required.
4. OPM Form 1 Gift and Campaign Contribution Certification. Please complete the required information on page 2. If no contributions were made, then please indicate as such by writing in the word, “none”.  Your signature and acknowledgement before Commissioner of the Superior Court or Notary Public is required.
5. OPM Form 7 Iran Certification. Please check applicable boxes on the form. Your signature and acknowledgement before Commissioner of the Superior Court or Notary Public is required.
