Prerequisites:- Two Health Club Contracts which the applicant is currently using or intends to use. Each contract submitted must include therein “Buyer’s Right to Cancel”, prices of all available memberships, and a list of equipment and services. SUBMIT PROPOSED CONTRACTS ONLY.
