How can I apply for DDS services?
Please submit this HIPAA form and the DDS application form to help us determine if you are eligible for services. We ask you to include copies of your birth certificate, Social Security card, medical insurance card, last three Individualized Education Programs (IEPs), and medical records. Please also submit autism test results if you are applying for services from the DSS' Autism Spectrum Disorder Program.
