How DDS Determines an Individual's Level of Need (LON)

Our case managers use a tool to evaluate your need for support and services called the CT Level of Need Assessment (LON).

The LON uses an app to create a profile based on your needs. The app also creates an individualized risk profile to help create your Individual Plan.

Case Managers complete a LON with the help of friends, family, and providers who know you well.
