Family members and next of kin can request a VA provided marker (Headstone, Niche Cover, or Bronze Medallion) for installation in a private cemetery.

1. The VA will provide a marker to the next of kin of an eligible veteran to mark the Veterans grave in a private cemetery.

• For a headstone, grave marker, or niche marker, the next of kin completes and submits VA Form 40-1330
• For a medallion to be placed on a privately purchased headstone or marker, the next of kin completes and submits VA Form 40-1330M

2. Please ensure the correct type of headstone, grave marker, or niche marker is selected.

3. Please note that the private cemetery director must sign the form indicating approval of the type marker ordered.

4. Items are shipped directly to the cemetery.
