For unvaccinated students to participate in Project COVID DeteCT, parents and guardians must fill out the Project COVID DeteCT Parent Consent Form and return a signed copy to the school. The signed consent form will cover the duration of the weekly testing program. Consent forms included below serve as examples and can be subject to change
