The following State Agencies receive complaints and address allegations of abuse, neglect, and exploitation.
- Children aged 0-17: Contact the Department of Children and Families Careline at 800-842-2288. You can also fill out the DCF-136 Form.
- Adults with an Intellectual or Developmental Disability ages 18 – 59: Contact the Department of Developmental Services Abuse Investigations Division 844-878-8923. You can also fill out the DDS PA-6 Form (Mandatory Reporters) – Email to: dds.aid@ct.gov.
- Adult Residents of Nursing Home Facilities Age 18+ (Mandated Reporters only): For non-urgent suspected abuse, neglect or exploitation: Complete form W-410 and fax to the Department of Social Services at 860-424-5091. You can also make an online referral.
- Connecticut citizens residing in nursing homes, residential care homes and assisted living communities: Contact the Long Term Care Ombudsman Program statewide toll-free number 1-866-388-1888 or local number 860-424-5200. You can also e-mail the LTCOP: ltcop@ct.gov.
Hospice and Medicare Home Health Hotline: For home health services complaints call 800-828-9769. - Patients/residents, regardless of age, for which there are concerns about the healthcare, treatment, or services they have received (or did not receive) at a Department of Public Health licensed healthcare facility: Contact the Department of Public Health though their online form FLIS Complaint Submission.
- Older Adults Aged 60+: For emergency or immediate response (within 24 hours) situations, contact the Protective Services for the Elderly hotline at 888-385-4225. Please call 911 for emergencies or life-threatening situations that must be dealt with immediately. You can also submit a written report PSE Report Form (W-675). Fill it out and email: PSEReferrals.DSS@ct.gov or fax: 860-424-5091.

The Coalition of Elder Justice, a multidisciplinary, statewide group of private and public stakeholders working together to prevent elder abuse and protect the rights, independence, security, and well-being of vulnerable elders in Connecticut. https://elderjusticect.org.
- The Commission on Women, Children, Seniors, Equity & Opportunity (CWCSEO) is required by State Law (Public Act No. 19-116) to provide a portal on the CWCSEO’s website with links to publicly available databases of persons found responsible for assault, abuse, neglect, exploitation or abandonment of elderly persons or persons with disabilities. The purpose is to provide a convenient location for site visitors to access this information. The list includes the National Sex Offender Registry, Connecticut Sex Offender Registry, CT Nurse’s Aide Registry, Practitioner/Entity Suspension or Exclusion Database, CT Criminal Motor Vehicle Conviction Database and the Professional Licensure Verification Database.
