for the development of a Continuous Residential Supports (CRS)
DDS is offering a Notice of Opportunity for a DDS qualified provider to manage, oversee and provide creative growth for a CRS program for three medically complex woman. Interested providers will need to be a DDS qualified vendor for Continuous Residential Supports
The three ladies have resided together for many years and would like to remain living together. The individuals’ teams support the women continuing to live together. Their current caregiver will soon be retiring. LPNs around the clock with RN oversight.
We are seeking a well-appointed living environment, communal areas for socializing and outdoor spaces designed for relaxation. The property identified will need to be wheelchair accessible. The home can be located anywhere within the state. The home should have opportunities to integrate technology to improve service delivery, communication, and accessibility.
