Reimbursement for the installation of Veterans' headstones in private cemeteries

The State of Connecticut will provide a partial reimbursement of the cost to install a Veteran Affairs-provided marker in a private cemetery.

  1. The vendor seeking reimbursement for the installation of the headstone (usually the cemetery or funeral director) must complete DVA Form CMS-3 (Request for Reimbursement) within TWO YEARS of the date of death of the eligible Veteran.

  2. Connecticut Veterans Affairs will review the application. Upon approval, a certification will be provided to the requestor.

  3. When the Veteran Affairs-provided marker is installed, the requestor submits the approved certification, a copy of the shipping document showing the headstone was received (or a photo of the monument), and a certification of installation. Payment must be requested within ONE YEAR of the date of certification.
