Application Processes for Restoration of State Benefits for Certain Veterans with an OTH Discharge

Qualifying Condition Verification Process and form for PTSD - TBI or MST

 

If you have military related post-traumatic stress disorder (PTSD) or traumatic brain injury (TBI), or experienced military sexual trauma (MST), and you have an OTH discharge, you may be eligible for state benefits using the Qualifying Condition Verification Process. To get started, you must obtain healthcare from a U.S. Department of Veterans Affairs or Vet Center healthcare provider, and they will need to review and sign the Qualifying Condition Verification Form certifying that you have a diagnosis of PTSD or TBI or that you have disclosed MST.

Veterans with OTH discharges can qualify for VA healthcare. If you have trouble accessing a VA healthcare provider to sign your form, you may contact CT DVA for assistance and referral information.

 

Option of Obtaining DVA Confirmation of Eligibility Letter In Lieu of Disclosing Qualifying Condition Verification Form When Applying for Benefits.

Applicants seeking restoration of benefits based on PTSD, TBI, or MST may use the Qualifying Condition Verification process established by Public Act 18-47 rather than filing with the EQRB. However, if a Veteran does not want to present the Qualifying Condition Verification Form to state or municipal agencies for the purposes of obtaining benefits because of the personal nature of information on that form, the Veteran may seek a DVA Confirmation of Eligibility Letter.

A DVA Confirmation of Eligibility Letter is optional and provided for the protection of veterans’ privacy. To get a DVA Confirmation of Eligibility Letter follow this process:

 

  1. Complete the Qualifying Condition Verification Form and have it signed by a VA Healthcare provider.
  2. Send the form, with a request for a DVA to generate a letter acknowledging benefit eligibility via email to: EQRB.dva@ct.gov
