If you aren’t happy with the quality of care you or a loved one received, contact the right team based on who your complaint is about:
- About a doctor, nurse, or other health professional: Contact the Department of Public Health Practitioner Investigations Unit
- About a hospital, clinic, office, or lab: Contact the Department of Public Health Facility Licensing & Investigation Section
- About a nursing home, residential care home, or assisted living community: Contact the Long Term Care Ombudsman
