The state uses the National Association of Insurance Commissioner’s (NAIC) SBS (State Based Systems), This is a web-based app with a wide range of resources for regulators and licensees. In SBS, you can visit the Licensing Quick Center to:

  • Lookup a License
  • Print a License
  • Print an Education Transcript
  • Print a Report
  • Apply for a License
  • Renew a License
  • Attach Documents to the Attachment Warehouse
  • Update Contact Information

Residency Change Request 

This form is only to be used if moving into or out of the state of Connecticut. Please complete the respective form and email the department at cid.licensing@ct.gov

Doing Business As (DBA) 

To change Doing Business As (DBA) please complete the respective form and email the department at cid.licensing@ct.gov

Letter of Certification 

Use this link if you have moved to another state and need proof of your standing in Connecticut.

Letter of Clearance Request 

Use this form if you are a previous Connecticut resident agent who is applying for a resident license in a new state.

Voluntary Surrender Request

Change Your Name

You can now change your address and name online by going to NIPR’s website:
1. Under the Licensing Center, select “Change Contact Info.”
2. Go to the Online Application.
3. Then select Product Type “Demographic Update.”  
If you are unable to do this online, please email the department at cid.licensing@ct.gov

Change Your Address

3. Then select Product Type “Contact Change Request (Change Address, Phone, Email).”
