Knowing your support options can help you plan for you and your loved ones’ current and future needs. Through DSS, you can apply for healthcare coverage for stays at:
  • Skilled Nursing Facilities (SNF) and Intermediate Care Facilities (ICF)
  • Residential Care Homes (RCH) and Rated Housing Facilities (RHF)

Want to fill out the application on paper? Please mail an application packet with as much documentation as possible to your LTSS Application Center. You can find a list of towns assigned to each Application Center both in the application and below.
